Office Visits
• Guaranteed, on-time medical visits ensures efficient use of “your” time.

• All test results (even when normal) are communicated to you by mail or personally from your physician.

Options

Choose a level of access that suits your needs:

Option A gives you:

Access to Physician. You will be given the Physician’s cell phone number and e-mail address to communicate with the Physician. Telephonic consultations are available with the Physician twenty-four hours a day, seven days a week, three hundred and sixty-five days a year.

Same/Next Day Appointments. To the extent you require in-person healthcare services, the Physician will meet with you at the Physician’s medical office on a same-day or next-day basis. In-person meetings will be mutually scheduled and conducted between the hours of 8:00 AM to 5:00 PM, Monday through Friday, and at mutually agreed upon times on Saturdays and Sundays.

Option A Pricing:

(This plan offers the same access as Option B, with the exception of house or place of business visits.)
This option requires that you visit our medical office for appointments.

- Individual: $2,500 annually.
- Couple: $4,500 annually.
- Family: Children may be added for $1,000 each.
- Elderly Parents: Individually $2,500 annually; Couple $4,500 annually.

Option B Pricing:

(Option B gives you the same access as Option A except that Option B also gives you private, convenient, and on-time medical visits conducted in your home or place of business.)

- Individual: $5,000 annually.
- Couple: $9,000 annually.
- Children may be added for $2000 each.
- Elderly Parents: $5,000 annually, or $9,000 per couple.

A 5% discount is applied when the yearly fee is pre-paid.
